The Tennessee Department of Health requires proof of the following immunization be submitted to Heritage Middle School Clinic for your Seventh Grader for the 2026-2027 school year.


Tdap (Tetanus-diptheria-pertussis booster)


The above immunization must be submitted on a “Tennessee State Immunization Information System Certificate (TennIIS form).” These forms are available at your child’s doctor’s office, the Health Department or a walk-in clinic.



*Please send the completed form to the School Nurse at Heritage Middle. The immunization record deadline is October 13, 2026. Please have the immunizations completed as soon as possible.

Snapcodes were sent to HMS families several days ago via email. Please verify that you have received these to complete your returning student registration for the 26-27 School Year.

Heritage Middle School girl's soccer tryouts will be August 3rd and 4th from 4-6pm. Girls that are trying out will need a physical dated after April 15, 2026 and a completed sports packet turned in to Coach Hamilton. Sports packets will be available at Chromebook Pick-up Night and during the first several days of school.

If you completed last year as a Blount County enrolled student from a feeder school (Townsend, Walland, Prospect, Porter or Montvale) or were an HMS student last year, then you do NOT need to attend registration. You will only need to re-enroll with your SNAPCODE which will be sent out by email the week of July 20th. (Please check your junk email if you do not see it in your regular email.)
