HMS parents and guardians,
This week, your child will be coming home with a dental sealant program permission form. The dental hygienist will begin screening the first week of November. Anyone can take advantage of this program, even if your child regularly sees a dentist. The best thing about this program is that it is completely FREE! Simply fill out the short health history and have your student return it to the clinic or to their teacher by October 29th.
I hope you take advantage of this wonderful opportunity.
Nurse Nicole
Sixth- and eighth-grade parents,
Your student will be coming home with an opt-out form for health screenings. The health screening will take place during school hours on November 16-17. Please return the form only if you do NOT want your student to participate.
Thank you!
